Job summary

Infinity Hospice Care LLC in Graham, Texas, seeks an experienced Registered Nurse to administer home health care for diverse patients, coordinate with the interdisciplinary team, and support families.

You will perform initial and ongoing assessments, develop and adjust care plans, document accurately, and participate in case conferences while maintaining compliance and timelines.

This role requires current RN license, CPR, reliable transportation, and the ability to travel locally.

Responsibilities

• Promote the Company's mission, vision, and values.
• Complete initial and ongoing assessments, identifying physical, psychosocial, and environmental needs, including OASIS.
• Regularly reevaluate nursing needs and outcomes, assist in recovery/maintenance of life functions, prevent illness.
• Develop, initiate, and revise the Plan of Care to ensure quality and continuity of care.
• Provide effective communication to clients, families and health professionals; participate in Case Conferences.
• Coordinate infection control and universal precautions in client care activities to protect clients and staff.
• Coordinate changes in client condition with physician and team; inform accordingly.
• Monitor cases for third-party payer compliance.
• Document timely to ensure policy compliance.
• Supervise and provide clinical direction to other nursing personnel.
• Perform on-call duties as assigned.
• Engage in in-service programs and maintain competency.
• Meet mandatory continuing education requirements.
