Job summary

Infinity Hospice Care LLC is seeking a Registered Nurse for home health in Texas. The RN will administer nursing care in clients' homes, coordinate with the interdisciplinary team, and support patients and families, ensuring safety and quality of care.

Requirements include 2+ years RN experience in behavioral health, preferred home health exposure, active RN license, CPR, and a valid driver's license. Strong communication and planning skills are essential for success in this role.

Qualifications

• RN must be licensed and in good standing.
• Minimum 2 years RN experience in behavioral health.
• Home health experience preferred.
• Current CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Promote and embody the company mission, vision, and values.
• Complete initial and ongoing assessments of home health clients (including OASIS).
• Reevaluate nursing needs and outcomes; assist patient recovery or maintenance.
• Develop and revise the Plan of Care to ensure quality and continuity.
• Communicate effectively with clients, families, and healthcare team; participate in Case Conferences.
