Job summary

Qualifications

• Must hold an LVN/LPN license in good standing in TX or applicable states.
• Minimum 1 year of clinical LVN/LPN experience, home health preferred.
• Must read, write and comprehend English.
• Ability to travel locally to patients' homes.
• Current CPR certification and valid driver's license.

Responsibilities

• Administer nursing care under RN supervision in patients' homes.
• Coordinate care with interdisciplinary team, patients, and families.
• Plan and implement care per Plan of Care (POC).
• Provide infection control and maintain high-quality patient care.
• Communicate effectively with clients, families, and healthcare professionals.
• Document timely per agency policy and complete case conferences.
• Monitor cases for third-party payer requirements and compliance.
• Demonstrate ongoing professional growth and continuing education.
• Travel to patient locations and manage flexible schedule.
