Elite Home Health Group INC in Clermont, FL is seeking a Physical Therapist Assistant (PTA) to provide skilled physical therapy services in patients' homes under the supervision of a licensed Physical Therapist. The role focuses on restoring mobility, improving strength, and promoting independence in a home health setting.

Responsibilities include delivering therapy per plan of care, documenting progress in EMR, complying with Medicare/state policies, and collaborating with nurses and physicians

Qualifications

• Graduate of an accredited PTA program.
• Current state PTA licensure in good standing.
• CPR certification required.
• Valid driver’s license and reliable transportation.
• Minimum of one year of PT experience; home health preferred.

Responsibilities

• Provide skilled physical therapy services under the supervision of the Physical Therapist (PT).
• Deliver therapy per physician orders and the established therapy plan of care.
• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and mobility restoration.
• Support fall prevention, strengthening, and safe functional movement in the home setting.
• Observe and report patient response to treatment, progress, and changes in condition to the supervising PT.
• Communicate patient concerns, safety issues, and therapy barriers promptly.
• Reinforce patient and caregiver education as directed by the PT.
• Complete timely and accurate treatment documentation, progress notes, and visit records in the EMR system.
• Ensure documentation supports medical necessity and therapy continuity.
• Maintain compliance with Medicare requirements, state regulations, and agency policies.
• Work closely with the PT, nurses, physicians, and interdisciplinary care team to support continuity of care.
• Participate in care conferences, discharge planning, and required staff meetings.
• Attend required in‑services, competency reviews, and professional development activities
