Job summary

Elite Home Health in Florida seeks dedicated Physical Therapist Assistants to support patients in their homes, promoting mobility, independence, and safe daily function under the supervision of a licensed Physical Therapist.

As part of an interdisciplinary team, you will deliver therapy per physician orders, document progress in the EMR, and uphold Medicare and agency policies while traveling within the assigned territory.

Qualifications

• Graduate of an accredited PTA program; current state licensure in good standing.
• Minimum 1 year experience in physical therapy or related clinical setting; home health preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to pass background screening and all compliance requirements.

Responsibilities

• Provide skilled physical therapy in home settings under PT supervision.
• Deliver interventions per physician orders and plan of care.
• Assist with exercises, gait and transfer training, and mobility restoration.
• Document treatment notes promptly in EMR.
• Coordinate with PT, nurses, physicians and interdisciplinary care team for continuity.
