Job summary

Elite Home Health Group INC seeks a dedicated Physical Therapist Assistant to provide in-home therapy services in Lake City, FL, under the supervision of a licensed Physical Therapist. You will help patients regain mobility and independence through hands-on treatments delivered in patients' homes.

The PTA performs interventions per physician orders within the plan of care and maintains detailed documentation while collaborating with nurses and physicians to ensure continuity of care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant (PTA) program required.
• Current state PTA licensure or certification in good standing.
• Minimum of one (1) year of experience in physical therapy or related clinical setting preferred; home health experience preferred.

Responsibilities

• Provide skilled physical therapy services under the direction of a licensed Physical Therapist.
• Deliver therapy interventions per physician orders and plan of care.
• Assist patients with therapeutic exercises, gait training, balance activities, and mobility restoration.
• Support fall prevention and safe functional movement in the home.
• Document treatments accurately in the EMR and communicate progress to the PT.
