Job summary
Elite Home Health Group INC is seeking a dedicated Physical Therapist Assistant (PTA) to provide skilled therapy in patients' homes in the Gainesville, FL area. The PTA will work under the direction of a licensed Physical Therapist to implement plans of care and help patients regain mobility and independence.

Qualifications
- Graduate of an accredited PTA program.
- Current Florida PTA license/certification in good standing.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Home health experience is a plus.
Responsibilities
- Provide PTA services under supervision of the supervising PT.
- Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and mobility training.
- Support strengthening, fall prevention, and safe functional movement.
- Monitor patient response to treatment and report progress or concerns to the PT.
- Reinforce patient and caregiver education as directed by the PT.
- Complete accurate and timely documentation in the EMR.
- Communicate with the PT and interdisciplinary care team regarding patient needs.
- Participate in care coordination, meetings, in-services, and required training.
- Follow Medicare requirements, state regulations, and agency policies.
