Job summary

Elite Home Health Group INC is seeking a dedicated Physical Therapist Assistant (PTA) for home health in Lakeland, FL. The PTA provides skilled physical therapy to patients in their homes under a licensed PT, working within a defined plan of care.

Responsibilities include implementing therapy plans, assisting with mobility and safety, and coordinating with the interdisciplinary team to support patient outcomes.

Qualifications

• Graduate PTA program completed and licensed in Florida; 1 year of PT experience preferred, home health preferred.
• Current CPR certification required.
• Valid driver's license, reliable transportation and auto insurance.
• Ability to pass background screening and compliance requirements.

Responsibilities

• Provide physical therapy services under the direction of the supervising Physical Therapist.
• Deliver therapy interventions per physician orders and plan of care.
• Assist patients with exercises, gait training, transfers, balance, and mobility.
• Support fall prevention and safe home mobility; document progress and changes.
• Maintain timely and accurate treatment notes and EMR entries.
