Job summary

Elite Home Health Group INC is seeking a dedicated Physical Therapist Assistant (PTA) to provide skilled therapy in patients' homes in the Gainesville, FL area. The PTA will work under the direction of a licensed Physical Therapist to implement plans of care and help patients regain mobility and independence.

Qualifications

• Graduate of an accredited PTA program.
• Current Florida PTA license/certification in good standing.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Home health experience is a plus.

Responsibilities

• Provide PTA services under supervision of the supervising PT.
• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and mobility training.
• Support strengthening, fall prevention, and safe functional movement.
• Monitor patient response to treatment and report progress or concerns to the PT.
• Reinforce patient and caregiver education as directed by the PT.
• Complete accurate and timely documentation in the EMR.
• Communicate with the PT and interdisciplinary care team regarding patient needs.
• Participate in care coordination, meetings, in-services, and required training.
• Follow Medicare requirements, state regulations, and agency policies.

We are seeking a dedicated Physical Therapist Assistant (PTA) to provide skilled therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist (PT).

Position Summary

The PTA provides skilled physical therapy treatments in accordance with physician orders, the established therapy plan of care, PT direction, agency policies, and applicable regulations.

• Valid driver's license, reliable transportation, and auto insurance
• Ability to meet all required background and compliance requirements
