Job summary

The North Country Home Health & Hospice Agency seeks a Physical Therapist Assistant to provide in-home physical therapy under the supervision of a Physical Therapist. You will use therapeutic exercise, modalities and activities to maximize functional outcomes in the home setting.

You will join a leadership team focused on high-quality patient outcomes, collaborating with medical, clinical and support staff.

Qualifications

• Associate degree from an accredited PTA program.
• Current NH PTA license required.
• Strong organizational and multitasking abilities.
• Excellent customer service and communication skills.
• Proficient with Windows-based computers and software.
• BLS certification required.

Responsibilities

• Performs physical therapy procedures in the client's home under supervision.
• Administers therapeutic exercises and modalities as prescribed.
• Instructs patients and families on functional activities and device use.
• Maintains accurate progress notes and SOAP documentation.
• Ensures equipment is safe and reports any unsafe items.
• Collaborates with the supervising Physical Therapist and Clinical Nurse Manager.
