Job summary

CenterWell Home Health is seeking a dedicated Home Health Physical Therapist Assistant to help patients regain strength, mobility and independence in their own homes. You will collaborate with a Physical Therapist to deliver personalized care and document progress for ongoing treatment planning.

Required qualifications include a current state PT license, valid driver's license, CPR certification, and English fluency.

Qualifications

• Licensed Physical Therapist Assistant; current state licensure to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, lack of supplies, or staff or client changes.
• Apply infection control and universal precautions in patient care activities.
