Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to help patients regain strength and independence in their homes. You will deliver the prescribed therapy plan, perform exercises, document progress, and coordinate with the Physical Therapist and Branch Director.

This role requires a valid PT license, CPR, and reliable transportation. Strong collaboration with healthcare professionals, English fluency, and knowledge of geriatric changes are essential.

Qualifications

• Must hold a current state physical therapy license.
• CPR certification must be current.
• Valid driver’s license and reliable transportation.
• Experience collaborating with a healthcare team.
• Knowledge of geriatric physical, emotional, and social changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
• Apply concepts of infection control and universal precautions in performing patient care activities.
