Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver in-home therapy under the supervision of a Physical Therapist, helping patients regain mobility, strength and independence in their own homes. You will administer treatment, document records, attend meetings, and follow infection control practices while coordinating with a healthcare team.

The role requires licensure, CPR, a valid driver’s license, and English fluency, with a salary range aligned to full-time

Qualifications

• Current state license to practice Physical Therapy
• Current CPR certification
• Valid driver's license and reliable transportation
• Experience collaborating with a team of healthcare professionals
• Comprehensive knowledge of geriatric population
• Read, write, and speak fluent English

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes as developed by the Physical Therapist.
• Administer non-complex active and passive manual exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supplies, or staff or client changes.
• Apply infection control concepts and universal precautions in patient care.
