Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will implement the plan of care, perform prescribed exercises and modalities, and help patients regain function and independence in a home setting.

The role requires a current state PTA license, CPR, valid driver’s license, and ability to collaborate with a care team. 40 hours weekly with benefits and a focus on geriatric care.

Qualifications

• Fluent English required (read, write, speak).
• Current state license to practice Physical Therapy required.
• Valid driver’s license and transportation required.
• CPR certification required.
• Experience collaborating with healthcare professionals preferred.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes, as developed by the Physical Therapist.
• Perform non-complex manual therapy, therapeutic exercises, modalities as directed.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment issues or changes.
• Apply infection control and universal precautions in patient care.
