Job summary

CenterWell Home Health is seeking a dedicated Home Health Physical Therapist Assistant to join our patient-centered care team. You will administer the physical therapy plan in patients' homes, supporting functional improvement and independence with the supervising Physical Therapist.

You should have an active PTA license in the state, CPR certification, reliable transportation, and strong teamwork skills to coordinate care with clinicians.

Qualifications

• Active Physical Therapist Assistant license in the practicing state.
• Current CPR certification and valid driver's license.
• Experience collaborating with healthcare professionals and geriatric patients.

Responsibilities

• Administer the physical therapy treatment plan in patients' homes as developed by the Physical Therapist.
• Deliver non-complex active and passive manual therapies and modalities as prescribed.
• Document client records and submit weekly to the office.
• Attend educational meetings and coordinate department in-services.
• Notify supervising staff of equipment or supply issues and changes.
• Apply infection control and universal precautions in care delivery.
