CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver individualized therapy in patients' homes, reporting to the Clinical Manager. You will implement the PT plan, support functional improvements, and help clients regain strength, mobility and independence in a home-setting.

The role requires an active PT license, valid driver's license and CPR, plus ability to work with a multidisciplinary team and communicate fluently in English.

Qualificações

• Current state license to practice Physical Therapy.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Fluent reading, writing and speaking English.
• Knowledge of geriatric population changes.

Responsabilidades

• Administer the physical therapy treatment plan to patients in their homes.
• Perform non-complex manual therapy and modalities as directed.
• Document client records and submit documentation weekly.
• Attend meetings and coordinate department in-services.
• Notify supervisor of equipment or supply issues.
• Apply infection control and universal precautions in care activities.
