Job summary

CenterWell Home Health is seeking a Home Health Physical Therapy Assistant to deliver care in patients' homes under the supervision of a Physical Therapist. You will implement the treatment plan, guide therapeutic exercises, and help patients regain strength, mobility and independence in familiar surroundings.

The role requires an active state license, CPR, reliable transportation, and strong teamwork with clinicians.

Qualifications

• Current state license to practice Physical Therapy
• Valid driver’s license with reliable transportation
• Current CPR certification
• Experience collaborating with a team of healthcare professionals
• Comprehensive knowledge of geriatric physical, emotional, social and biological changes
• Read, write, and speak fluent English

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in patients' homes under the supervision of the Physical Therapist.
• Perform non-complex active and passive manual therapy, therapeutic massage, traction heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation to the office weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, supply shortages, or staff or client changes.
• Apply infection control concepts and universal precautions in patient care.
