Job summary

St. Mary’s Home Health Service, part of the LHC Group and Optum family, seeks a Per Diem Physical Therapist Assistant to deliver therapeutic exercise, patient education, and treatment within the established plan of care.

Responsibilities include guiding patients through active/passive exercises, documenting responses, and coordinating with the care team to optimize outcomes. Weekend availability and state licensure are required.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Follow all state laws governing provision of physical therapy in home care and adhere to supervising PT plan of care.
• Instruct and assist patients in active and passive exercise, gait, ADL, transfer, and mobility training.
• Observe, record, and report patient response to treatment and changes to supervising PT/physician.
• Educate patient, family, and care team in physical therapy procedures within scope of PTA.
• Function effectively in diverse home environments and with patients of all backgrounds.
