Job summary

St. Mary's Home Health Service, part of LHC Group and within the Optum family, seeks a Per Diem Physical Therapist Assistant to deliver treatment and education under the plan of care. The role emphasizes safe, effective therapy in home settings and supports patient well‑being.

Responsibilities include patient exercise instruction, gait and mobility training, and communication with the supervising PT and care team. Weekend availability is required, and a comprehensive benefits package is offered.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.

Responsibilities

• Follow state laws governing home care PT practice and implement the plan of care as defined by the supervising PT.
• Instructs patients in active and passive exercise, gait, ADL, transfer, and safety.
• Observe and report patient responses and changes to the supervising PT, coordinating with the healthcare team.
