Job summary

VitalCaring Group is seeking a Physical Therapy Assistant (PTA) to deliver hands-on home health care in patients’ homes. You’ll work one-on-one to help regain mobility, strength, and independence under the supervision of a licensed Physical Therapist.

You’ll educate patients and caregivers, document progress in the EMR, and collaborate with nurses and therapists to ensure coordinated care. This is a field-based role emphasizing autonomy with support.

Qualifications

• Graduate of an accredited PTA program and valid PTA license.
• Experience in home health is preferred.
• CPR certification required.

Responsibilities

• Provide direct physical therapy services under the direction of a licensed Physical Therapist.
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility.
• Support patients with transfers, strengthening, and activities of daily living.
• Educate patients and caregivers on exercises, safety, and use of assistive devices.
• Maintain accurate EMR documentation and communicate with the PT and care team.
