Job summary

VitalCaring Group is seeking a Field-Based Physical Therapy Assistant (PTA) to deliver direct physical therapy in patients’ homes. You will implement customized care plans to improve mobility, reduce pain, and restore function, while promoting independence and safety for clients in a home setting.

You will work closely with a licensed Physical Therapist, nurses, and care teams to coordinate care. A PTA license, CPR certification, and reliable transportation are required, with 1+ year of home

Qualifications

• Graduate of an accredited PTA program.
• Current PTA license or certification as required by state law.
• 1+ year of home health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct physical therapy services under the direction of a licensed Physical Therapist.
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, and functional mobility.
• Support patients with transfers, strengthening, and activities of daily living.
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate.
• Educate patients and caregivers on exercises, safety, and use of assistive devices.
