Job summary

AccentCare, Inc. is seeking a Licensed Physical Therapy Assistant (PTA) to join our Home Health team in the San Ramon area.

Under the supervision of a licensed Physical Therapist, you will provide patient care across a defined caseload and ensure documentation meets policy standards. The role requires an associate degree in physical therapy, a valid PTA license, CPR certification, and a willingness to travel to Pleasanton, Livermore, Danville and surrounding locations.

Qualifications

• Must have an associate degree in Physical Therapy from an accredited program.
• Valid PTA license, registration, or permit in the state of operation.
• CPR certification must be current.
• Licensed driver who can travel to all business locations.
• Ability to implement care plans and communicate findings to the Physical Therapist and physician.
• Ability to educate patients and families in safety and use of medical equipment.

Responsibilities

• Provide patient care and document treatment per physician orders and policy.
• Document interventions, approaches, and priorities for treatment.
• Educate and counsel patients and families during treatment.
• Present patient status to supervising therapist and team.
• Provide skilled treatment within agency and payer guidelines.
• Collaborate with team on treatment planning and intervention.
• Accurately document and submit daily notes and forms promptly.
• Meet agency productivity standards and manage time efficiently.
• Attend agency meetings and committees as assigned.
• Negotiate patient participation in treatment and scheduling.
