Job summary

Alternate Solutions Health Network, LLC is seeking a Licensed Physical Therapist Assistant (PTA) to provide skilled services in patients' homes across Blue Rock, Duncan Falls, and surrounding areas. The PTA will educate patients and families, ensure safety, and maintain accurate documentation under the supervision of a Physical Therapist.

The role requires at least one year of PTA experience, preferably in home care, and a valid driver's license.

Qualifications

• PTA with current licensure in the employment state.
• Minimum of one year PTA experience; home care experience preferred.

Responsibilities

• Provides skilled home care services as prescribed by the Physical Therapist plan.
• Observes and reports patient progress, reactions, and changes.
• Documents treatment, assessments, and education in a timely manner.
• Communicates with physician, therapist, and caregivers to coordinate care.
• Drives to patient locations and participates in on-call requirements.
