Job summary

VitalCaring is seeking a Physical Therapy Assistant (PTA) for field-based home health visits. You’ll work one-on-one with patients to help regain mobility, strength, and independence in the comfort of their homes.

As part of a patient-centered team, you’ll follow individualized plans of care, document progress in the EMR, and collaborate with therapists, nurses, and care teams to ensure coordinated, high‑quality treatment.

Qualifications

• Graduate of an accredited PTA program.
• Current PTA license or certification as required by state law.
• 1+ year home health experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct physical therapy services under the direction of a licensed Physical Therapist.
• Follow individualized plans of care to help patients reduce pain, improve mobility, and restore function.
• Assist patients with therapeutic exercises, gait training, balance, and activities of daily living.
• Support patients with transfers, strengthening, and activities of daily living.
• Utilize therapeutic modalities such as heat, cold, ultrasound, and electrical stimulation as appropriate.
• Educate patients and caregivers on exercises, safety, and use of assistive devices.
