Job summary

MyCHN Home Health Services is seeking a dedicated Physical Therapist Assistant (PTA) to provide in-home physical therapy under the supervision of a licensed Physical Therapist. The PTA will help patients improve mobility, strength, balance, and independence through individualized plans of care.

The ideal candidate will have graduated from an accredited PTA program and hold a Texas PTA license, with strong communication and documentation skills.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Texas Physical Therapist Assistant license in good standing.
• Knowledge of accepted physical therapy practices and treatment techniques.
• Ability to work under supervision of a licensed Physical Therapist.
• Excellent communication, interpersonal, and patient education skills.

Responsibilities

• Provide physical therapy treatments and interventions as outlined in the Plan of Care established by the supervising Physical Therapist.
• Assist patients with therapeutic exercises, gait training, balance activities, endurance training, functional mobility activities, and other prescribed treatments.
• Observe, monitor, and document patient responses to treatment and communicate findings to the supervising Physical Therapist.
• Recognize changes in patient condition and promptly report concerns to the Physical Therapist and interdisciplinary care team.
• Educate patients, families, and caregivers on prescribed therapeutic exercises, home exercise programs, safety precautions, and techniques to support recovery and independence.
• Reinforce patient goals and encourage adherence to treatment plans.
• Assist with the evaluation and recommendation of durable medical equipment (DME), adaptive devices, and assistive equipment as directed by the Physical Therapist.
• Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated patient care.
• Maintain accurate, complete, and timely clinical documentation in accordance with agency policies and state and federal regulatory requirements.
• Apply infection control practices and promote patient safety during all visits.
• Participate in required continuing education, in-service training, and professional development activities.
