Job summary

Guardian Angel Home Care Inc. is seeking compassionate Physical Therapist Assistants (PTAs) to join our Home Health team.

The PTA works under a licensed Physical Therapist to deliver medically prescribed therapy in patients' homes, aiming to improve mobility, strength, independence, and quality of life. Ideal candidates hold an accredited PTA program credential and active PTA license, with 1 year of clinical experience preferred.

Qualifications

• Graduate of a Physical Therapist Assistant program accredited by the APTA and/or AMA.
• Current PTA license in the state of employment.
• Minimum one (1) year of recent clinical experience preferred.
• Home Health experience preferred but not required.

Responsibilities

• Conduct prescribed therapy programs including exercise, massage, heat, water, light, and electrical modalities.
• Assist patients with active and passive therapeutic exercises.
• Record and report patient progress and response to treatment.
• Collaborate with supervising Physical Therapists and care teams.
• Prepare and maintain accurate clinical documentation.
• Discharge planning coordination with the therapy case manager.
