Job summary

Heritage Home Health and Hospice in Toledo, OH, is seeking a licensed Physical Therapist Assistant (PTA) to provide skilled physical therapy services in patients' homes under the supervision of a Physical Therapist. The role focuses on improving mobility, balance, and independence.

Qualifications

• Graduate of an accredited PTA program and active Ohio PTA license.
• Home health experience preferred but not required.
• Excellent communication and organizational skills.

Responsibilities

• Provide skilled PTA services under supervision of a licensed PT.
• Develop and implement treatment plans to improve mobility and independence.
• Monitor progress and communicate changes to the supervising PT.
• Educate patients and families on exercises and safety at home.
• Document visits and outcomes in the EHR.
• Coordinate with the interdisciplinary care team to ensure quality care.
