Job summary

Interim HealthCare Inc. seeks a Home Health Physical Therapist Assistant in Globe, AZ. This role supports patients in their homes, providing personalized therapy and advancing mobility and function on a flexible schedule.

You will work with an interdisciplinary team, under a supervising Physical Therapist, to implement the plan of care and educate patients and families on home-based exercise programs. Competitive benefits apply for full- and part-time roles.

Qualifications

• Requires PTA licensure in Arizona and graduation from an accredited PTA program.
• Strong interpersonal and communication skills are essential.
• Ability to document exercises accurately and educate patients and families.

Responsibilities

• Provide home-based physical therapy to patients who have mobility, strength, or function limitations.
• Collaborate with home health team (RN, LPN, CNA, HHA, OT, SLP) to implement the plan of care.
• Perform under the supervision of the treating Physical Therapist and within the care plan.
• Assist in documenting exercises and printing home-use routines.
• Educate patients and families with home exercise programs under supervision.
