Job summary

Sunrise Health Services is seeking a dedicated Physical Therapist Assistant (PTA) to support patients in home health in Arizona. The PTA will work under a licensed Physical Therapist, implementing individualized treatment plans to improve strength, mobility, and functional independence in patients’ homes.

The role offers flexible scheduling and both full-time and part-time options, with collaboration across an interdisciplinary team to ensure optimal patient outcomes.

Qualifications

• Must be a graduate from an accredited Physical Therapy program and licensed by the state of Arizona.
• Ability to implement individualized treatment plans under PT direction.
• Excellent communication and collaboration with the interdisciplinary care team.

Responsibilities

• Assist in evaluating the patient’s level of function.
• Assess home environment for safety hazards and equipment needs.
• Identify functional problems and establish a plan of care to improve function.
• Perform ROM/MMT, gait analysis, and document findings.
• Complete Start of Care and ongoing assessments per agency policy.
• Deliver prescribed therapy and educate patients and caregivers; coordinate with the care team.
• Document patient care and ensure timely submission of records.
