Job summary

Mercy Medical Center Home Health and Hospice is seeking a licensed Physical Therapist Assistant to provide skilled home health services under the direction of a Physical Therapist. The role covers a wide service area with flexible scheduling and a supportive team environment.

The PTA will educate patients and families, document care, and help patients regain function using exercises, manual techniques, and modalities in the home setting.

Qualifications

• Physical Therapist Assistant with a current license in the state of employment.
• Minimum of one year of experience as a PTA in an acute setting is required.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Valid driver’s license and auto insurance with your name as a listed driver.

Responsibilities

• Provides skilled PTA services in patients’ homes following the Physical Therapist plan of care.
• Observes, records, and reports patient progress and reactions.
• Performs PT procedures and educational activities prescribed by the PT.
• Educates patients and families on therapy techniques and safety.
• Documents treatment and maintains regulatory compliance.
• Coordinates with schedulers and participates in case conferences.
• Drives to patient locations per scheduled visits.
