Job summary

Qualifications

Responsibilities

• Assist with development and implementation of the plan of care within defined goals.
• Collaborate with interdisciplinary team to manage patient care plan.
• Demonstrate competency in home health patient care skills.
• Meet agency productivity standards and manage time/resources efficiently.
• Complete timely and accurate documentation at the point of care.
• Assist PT in assessing patient function and disability levels.
• Maintain licensure, certifications, and clinical records per regulations.
• Support surveys and required reports.
