Job summary

PruittHealth is seeking a Physical Therapy Assistant (PTA) to provide home health services under supervision in patients' homes. You will implement the plan of care, document progress, and collaborate with the PT and care team to support mobility and independence.

This role requires a PTA degree, an active state license, and a valid driver's license with access to reliable transportation. Prior home health experience is preferred; travel to York, Chester, and Lancaster areas may be needed.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• Foreign-trained: the program must be deemed substantially equivalent to a PTA entry level education in the U.S. by a credentials evaluation organization approved by the APTA.

Responsibilities

• Assist with the development and implementation of the plan of care within goals defined by the physician, patient/patient family and interdisciplinary team.
• Help maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan of care.
• Demonstrate intermediate knowledge of home care and competency in discipline for specific patient care skills, required for the provision of care.
• Meet agency productivity standards and utilize time and resources effectively and efficiently.
• Complete documentation timely, accurately, and at the point of care, according to industry standards.
• Assist the physical therapist in assessing the physical disability and level of function of patients.
• Maintain current license, certification, and clinical records to meet federal, state and agency regulations and guidelines
• Participate in center/agency surveys (Licensure / JCAHO), and any subsequently required reports.
