Job summary

PruittHealth Home Health seeks a Physical Therapy Assistant (PTA) to provide home-based therapy to patients in their homes under a supervising physical therapist. The role emphasizes patient-centered care, documentation, and collaboration with the care team.

Qualifications

• Minimum of 2 years' therapy experience.
• Must have a valid, unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• If foreign-trained, the program must be deemed substantially equivalent to a PTA entry level education in the U.S.

Responsibilities

• Assist with the development and implementation of the plan of care within goals defined by the physician, patient/patient family and interdisciplinary team.
• Collaborate with the interdisciplinary team to develop, integrate, and manage the patient care plan.
• Demonstrate competency in home care and patient- specific skills.
• Meet agency productivity standards and manage time/resources effectively.
• Complete documentation timely at the point of care following industry standards.
• Assist the physical therapist in assessing patient function.
• Maintain current license, certification, and clinical records as required.
• Participate in surveys and required reports.
• Report any violations to supervisor and resolve issues.
• Be self-directed, prioritize tasks, and meet deadlines.
