Job summary

Qualifications

• Must hold a valid, unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• Foreign-trained graduates must be deemed substantially equivalent by a credentials evaluation organization approved by APTA.

Responsibilities

• Assist with development and implementation of the plan of care within defined goals.
• Collaborate with interdisciplinary team and supervising physician to manage patient care plan.
• Demonstrate competency in home care patient skills and maintain documentation at the point of care.
• Maintain licensure, certifications and clinical records to meet regulations.
• Assist in patient assessments of physical function and progress.
