Job summary

Southwest General Home Health is seeking a licensed Physical Therapist Assistant to provide skilled services in patients' homes across the Parma, Independence, and greater Cleveland area. You will educate patients and families, document progress, and coordinate with the physical therapy team to maintain high-quality care.

The role requires good communication, reliability, and adherence to Medicare guidelines. A valid driver’s license and access to a vehicle are needed to travel between sites.

Qualifications

• Licensed Physical Therapist Assistant (state).
• Minimum of 1 year PTA experience in acute setting preferred.
• Ability to communicate effectively with patients, families and co-workers.
• Knowledge of therapy best practices and ability to make PT judgments.
• Detail-oriented, disciplined and timely in documentation.

Responsibilities

• Provides skilled PTA services in patient homes per the plan of care.
• Observes and documents patient progress and changes.
• Delivers PT modalities to relieve pain and improve function.
• Educates patients and families on therapy techniques and safety.
• Completes clinical documentation per Medicare/Federal guidelines.
• Communicates with scheduling and care team as needed.
