Job summary

Cleveland Clinic’s Home Care and Hospice network seeks a dedicated Physical Therapy Assistant to administer patient-centered therapy across home visits in the Greater Cleveland area. You will support skilled rehabilitation under a licensed Physical Therapist, observe patient progress, and document responses.

This full-time role requires an Associate’s or PTA program degree, current PTA licensure, BLS, and a valid driver’s license with insurance coverage for Homecare visits.

Qualifications

• Associate's degree or certificate in physical therapy assistant.
• Graduate of PTA program accredited by the American Physical Therapy Association.
• Current state licensure as a Physical Therapy Assistant (PTA).
• Basic Life Support (BLS) certification.
• Valid driver's license with proof of adequate auto insurance for homecare.

Responsibilities

• Administer patient-centered physical therapy treatment under supervision.
• Observe, document and communicate patient progress and response to treatment.
• Plan and implement individualized therapeutic programs with patient/family.
• Provide education on physical therapy topics to patients and caregivers.
• Act as clinical instructor or participate in student education and evaluation.
• Serve as a resource for volunteers and observers.
