Job summary

Cleveland Clinic is seeking a dedicated Physical Therapy Assistant to deliver patient-centered home care across Cleveland and surrounding areas. The role runs Monday–Friday, 8:00am–5:00pm with occasional Saturdays and holiday weekends.

You will assess, document, and adapt therapeutic plans, educate patients and families, and collaborate with licensed clinicians to ensure skilled, evidence-based care in the home setting.

Qualifications

• Associate's degree or PTA certificate is required.
• Graduate of a PTA program accredited by the American Physical Therapy Association.
• Current state licensure as a Physical Therapy Assistant (PTA).
• Basic Life Support (BLS) certification through the American Heart Association.
• Valid Driver’s License with proof of $100,000/$300,000 coverage for Homecare.

Responsibilities

• Administer patient-centered Physical Therapy treatment under licensed PT supervision.
• Observe, document, and communicate patient progress and response to treatment.
• Plan and implement individualized therapeutic programs with patient and family.
• Provide education on Physical Therapy topics to patients and caregivers.
• Act as a clinical instructor or participate in education of students.
• Act as a resource for volunteers and observers.
