Job summary

Boost Home Healthcare is seeking a licensed Physical Therapist Assistant (PTA) to provide physician-prescribed physical therapy in patients' homes across the Northeast Ohio region. You will help restore function, minimize disability, and support overall recovery under clinical supervision.

Ideal candidates have an accredited PTA program graduation, current state licensure, and experience in home health. Travel and a valid driver’s license required; Lorain County residents preferred.

Qualifications

• Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association.
• Currently licensed in the state in which practicing.
• Two (2) years’ experience preferred in a home health setting.
• Knowledgeable and experience with Medicare OASIS documentation.
• Licensed driver with automobile insured in accordance with state/Agency requirements. Reliable transportation with good driving record.
• Preferably resides in Lorain County (Avon, Elyria, Lorain, Amherst, etc.)

Responsibilities

• Provide physician prescribed physical therapy.
• Improve or minimize residual physical disabilities of the patient.
• Return the individual to optimum and productive level within the patient’s capability.
• Participate with all other home care personnel in patient care planning.
• Documents all services provided, observations, client progress, and recommendations.
• Participate in interdisciplinary activities per Agency guidelines.
