Job summary

Adaptive Home Health is seeking a Physical Therapist (PT) to deliver in-home care in the Detroit area. You will manage a full caseload using our AI-native platform to streamline charting and documentation.

Join a team focused on exceptional patient care with a schedule that includes full-time weekdays and occasional weekends. Michigan license required and willingness to travel within the territory.

Qualifications

• Active Michigan PT license in good standing.
• Strong clinical judgment and ability to work independently in the field.
• Excellent communication and documentation skills.
• Willingness to travel daily across assigned territory.
• Ability to carry/transport basic therapy equipment.
• Competency in core home health PT scope including gait, transfers, balance, and home safety.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening, mobility, safety.
• Set measurable goals and track progress; adjust treatment as needed.
• Drive multidisciplinary coordination with care team and referents.
• Manage treatment outcomes; PTA supervision where appropriate.
• Coach patients and caregivers with practical home programs.
• Document promptly in a defensible, efficient manner.
