Job summary

Elite Home Health in Florida is seeking a licensed Physical Therapist Assistant (PTA) to provide in‑home physical therapy under the supervision of a licensed PT, following physician orders and the established plan of care.

The PTA will travel to patient homes, collaborate with an interdisciplinary care team, and document progress in the EMR to support patient mobility, safety, and independence while ensuring compliance with Medicare and state regulations.

Qualifications

• Graduate of an accredited Physical Therapist Assistant (PTA) program required.
• Current Florida PTA licensure in good standing required.
• Minimum of one year of physical therapy or related clinical experience preferred; home health experience preferred.
• Current CPR certification required
• Valid driver’s license, reliable transportation, and auto insurance
• Ability to pass background screening and all compliance requirements

Responsibilities

• Provide physical therapy treatments under the direction and supervision of the Physical Therapist (PT).
• Deliver therapy interventions in accordance with physician orders and the established plan of care.
• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and mobility restoration.
• Support fall prevention, strengthening, and safe functional movement in the home setting.
• Observe and report patient response to treatment, progress, and changes in condition to the supervising PT.
• Communicate patient concerns, safety issues, and therapy barriers promptly.
• Reinforce patient and caregiver education as directed by the PT.
