Job summary

The PTA will implement prescribed interventions and contribute to individualized care plans in a home health setting.

You will monitor patient progress, document outcomes, and collaborate with caregivers to support rehabilitation goals while adhering to state regulations and professional standards.

Qualifications

• Must be a graduate of a CAPTE-accredited PTA program.
• Current unrestricted PTA license or state certification.
• Current unrestricted Driver’s License.
• Therapy skills aligned with standard practice per APTA.
• Good interpersonal skills.
• Knowledge of durable medical equipment (DME) for PT patients.
• CPR certification is current.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Provide physical therapy services under the supervision of a licensed physical therapist.
• Implement selected components of patient interventions and treatments.
• Document patient progress and coordinate with caregivers and the care team.
