Job summary

ASSISTED HOME HEALTH INC is seeking compassionate physical therapists to provide home-based physical therapy across patient locations in Florida. The role requires a Florida-licensed PT with a Bachelor's or Master's in Physical Therapy and at least one year of healthcare experience.

A valid driver's license and ability to spend ~20% of the day driving are essential. You will assess, document, and coordinate with physicians and families, supervise a PTA, and participate in required meetings,

Qualifications

Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
Florida licensed Physical Therapist.
Minimum of one year healthcare experience.
Current driver’s license; ability to drive ~20% of day.
Commitment to deadlines for charting and time management.

Responsibilities

Provide physical therapy as prescribed, safely in the home, and assist physician in evaluating patients.
Observe and record activities and findings in the clinical record.
Report to the physician the patient’s response to treatment and changes in condition.
Instruct patient and family in care and use of physical therapy devices.
Instruct other health team personnel and family members in phases of therapy.
Instruct the family on the patient’s total physical therapy program.
Supervises the Physical Therapy Assistant.
Participate in case conferences, utilization reviews, orientations, in-services and CE meetings.
