Job summary

Memorial White Rose Home Health, part of LHC Group and Optum family, seeks a Physical Therapist Assistant to deliver treatment, provide exercise instruction and patient education within home care, following an established plan of care.

Under supervision of a Physical Therapist, you will guide active and passive exercises, coordinate with the care team, and help enhance patients’ well-being in diverse home settings. This role emphasizes compassionate, connected care and community service.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state-specific laws governing the provision of physical therapy in home care and adhere to the supervising PT's plan of care.
• Instructs and aids patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
• Observes, records, and reports patient responses to treatment and changes in condition to the supervising PT, nurse supervisor, and/or physician; coordinates care with the healthcare team as appropriate.
• Instructs patient, family, caregiver, and other health-care team members in therapy areas within the PTA scope.
• Function effectively in home environments regardless of age, race, creed, color, sex, disability, or financial condition.
