Qualifications

• Active Physical Therapist Assistant license in the state of work.
• At least 1 year of PTA experience delivering quality patient care.
• Current driver’s license and ability to travel to patient homes.

Responsibilities

• Provide compassionate, patient-centered PTA services in the home health setting under the direction of a licensed Physical Therapist.
• Support patients in restoring mobility, reducing pain, and maximizing functional independence through therapeutic exercises and mobility training.
• Educate patients and caregivers on safe use of mobility devices and monitor progress.
• Ensure compliant EMR documentation and communicate changes to the care team.
• Work independently while collaborating with the care team to ensure optimal outcomes.
• Travel to patient homes within the assigned territory.
