Job summary

Residential Home Health and Hospice in State College, PA seeks a dedicated Physical Therapist to deliver skilled home health care.

You will evaluate patients, develop therapy plans to improve strength and mobility, educate caregivers, and document care. Travel to patient homes within your territory is required. Bonus opportunities total up to $20,000, including a $10,000 first-year bonus and a $10,000 sign-on after 30 days.

Qualifications

• Active Physical Therapist license in the state you work.
• One year of experience delivering physical therapy in home health or clinical settings.
• Current driver's license and ability to spend ~20% of your day traveling to patient homes.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive patient evaluations and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises and safety at home.
• Document care thoroughly and ensure compliant EMR entries.
• Travel to patient homes within assigned territory.
