Job summary

Residential Home Health and Hospice in Mount Carmel, PA, is hiring a Physical Therapist to deliver skilled care in patients’ homes.

You will conduct comprehensive evaluations, develop therapy plans, educate patients and caregivers, and document in the EMR.

This field-based role includes travel within the assigned territory, a dedicated support team, and a first-year $10,000 success bonus.

Qualifications

• Active Physical Therapist license in the state you work
• One year of PT experience delivering patient care
• Valid driver’s license and ability to travel ~20% of workday

Responsibilities

• Deliver skilled, patient-centered care in a home health setting
• Perform comprehensive patient evaluations, mobility assessments, and develop individualized therapy plans
• Educate patients and caregivers on therapeutic exercises, injury prevention, and safe mobility at home
• Think critically, complete thorough EMR documentation
