Job summary

Residential Home Health and Hospice is seeking a Physical Therapist (PT) to provide skilled home health care in the Middleburg, PA area. This full-time role offers autonomy, meaningful patient relationships, and strong earning potential through bonuses.

You'll evaluate patients, develop therapy plans, educate families, and document in the EMR. Travel to patient homes is required, with a supportive team and growth opportunities.

Qualifications

• Active Physical Therapist license in the state you work (PA).
• One year of experience as a Physical Therapist.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Travel to patient homes within assigned territory.
