Job summary

Residential Home Health and Residential Hospice is hiring a Home Health Physical Therapist to deliver skilled care to patients in their homes in the Chambersburg, PA area. This role offers autonomy, meaningful patient relationships, and real career growth; this is where you belong.

We are offering a total bonus potential of $20,000, including a $10,000 first-year success bonus and an additional $10,000 sign-on bonus after 30 days in good standing.

Qualifications

• Active Physical Therapist license in the state you work.
• One year of experience as a Physical Therapist.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
