Interim HealthCare of Miami is seeking a Home Health Physical Therapist Assistant (PTA) to join our Miami-area team. You will provide home-based physical therapy, work with an interdisciplinary home health team, and help patients regain mobility and function in their own homes.

The role emphasizes individualized care, family coaching, and collaboration with care partners to implement and adjust the plan of care for optimal outcomes.

Competenze

• One year PTA experience, ideally in home healthcare.
• Active Florida PTA license required.
• CPR certification required.
• Knowledge of state and federal home health regulations.
• Good clinical judgment, strong interpersonal skills, dexterity and compassion.

Mansioni

• Provide home-based physical therapy to patients with mobility and functional challenges.
• Collaborate with the home health team (RN, LPN, CNA, HHA, OT, SLP) to implement the plan of care.
• Participate in development and evaluation of the plan of care with patients and caregivers.
• Assist patients with exercises and hands-on therapy to improve mobility and reduce pain.
• Educate patients on the plan of care, goals, exercises, and self-care.
• Coach families on plan of care and safe exercise techniques.
