Concierge Home Care is seeking a Home Health Physical Therapist Assistant (PTA) to deliver high-quality, one-on-one care in patients' homes in Jacksonville, FL. Qualified candidates hold an active Florida PTA license and a valid driver’s license; home health experience is a plus.

Qualifikationen

• Active Florida PTA license (unencumbered).
• Valid driver’s license with reliable transportation.
• Home health experience preferred, but not required.

Aufgaben

• Provide hands-on therapeutic care following the plan of care established by the supervising Physical Therapist.
• Assist patients in improving mobility, strength, and independence.
• Collaborate with patients, caregivers, and interdisciplinary teams.
• Monitor patient progress and communicate updates to the supervising PT.
• Complete documentation using AI-assisted tools.
